Please complete the form below to be added to our Nationwide Notary Database. In addition you will need to fax or email us the following:

  • Notary Commission
  • Bond and/or Insurance
  • Completed W-9 Form
  • Notary Contract (can be downloaded from our resource page)

Thank you for your interest in joining the Firma Solutions Team! There is a one time $18.00 application fee. This helps offset the fees incurred from the pay per click advertising on Yahoo and Google and also employee time to create, input and verify notary information. We will contact you upon receipt of your application and credentials.
